Step 1: Application Form:
This form will request information about your financial situation, the nature of your loss, and any supporting documentation such as medical bills, funeral expenses, or insurance details. Upon receiving your inquiry, a member of our support team will schedule an initial consultation. During and/or after the initial consultation we may ask for the follow documents that support your application. This may include, but is not limited to:
